Support Coordination:
Support Coordination is a service provided by Division employees or agencies under contract with the Division. Support Coordination assists individuals with disabilities and their families to develop service and support plans, based on the individual’s needs and wishes, and to coordinate and monitor the services and supports provided.

Support Coordination Standards:
The Division has just released a set of standards to define the way in which support coordination is to be offered. It is important that all support coordinators fully familiarize themselves with these.
