DSPD Eligibility
IMPORTANT: In order to begin the application process, please call our toll-free Intake number (1-877-568-0084) and ask to speak with an intake worker. Depending on disability type, age of onset and other factors, your assigned intake worker will help determine the appropriate waiver program for you to pursue, as well as the correct set of forms and other items that will be needed to determine eligibility for that particular waiver program. Your assigned worker will also let you know where to mail the forms, etc. Should you be in need of a translator DSPD will provide translation services.
